BIPS are indicated for the diagnosis of gastrointestinal obstructions and gastrointestinal motility disorders in dogs and cats. Specifically, BIPS are helpful in detecting physical obstructions (including partial obstructions), functional obstructions (ileus), delayed gastric emptying due to pyloric stenosis or gastric rhythm disturbances, overly rapid gastric emptying (dumping) and intestinal motility disorders.
When animals present with an acute onset of anorexia, vomiting and/or abdominal discomfort, the primary diagnostic aim in using BIPS is to rule out physical obstructions of the pylorus or bowel.
When animals present with chronic vomiting or diarrhea, the diagnostic aim in using BIPS is to detect delayed gastric emptying, gastric dumping, partial obstructions of the bowel, and intestinal motility disorders. Better sensitivity for the detection of these disorders is obtained if the BIPS studies are performed on fed rather than fasted patients. Because the type of food fed markedly affects gastric emptying rate, BIPS studies must be done with Hill's Prescription Diet d/d to be able to make comparisons with the normal values.
Administration of BIPS in a high fiber diet is more sensitive than administration of BIPS on an empty stomach for the diagnosis of partial obstructions of the intestinal tract with a luminal diameter of 5 mm or more. The fiber accumulates oral to the partial obstruction, arresting the passage of the markers. This results in delayed transit and persistent bunching of the markers in the abnormal intestinal segment (stagnant loop sign). Careful examination of the area of bowel containing the static markers may reveal an intestinal segment dilated by ingesta.
